Inclusion of a Third Patient Identifier on Paper-Based Orders to Increase Safety

To enhance patient safety, we are implementing a requirement for paper-based orders to include a minimum of three patient identifiers.

Effective June 30, 2025, all paper-based orders (including add-on orders) must include at least three patient identifiers. Patient’s Legal Name and Date of Birth must match the ones on the specimen’s label, and the third must match the identifier in the Electronic Health Record (EHR). Any extra identifiers on the specimen label must also match the ones on the paperwork and in the EHR.

Human Papillomavirus (HPV) Testing Update

Corewell Health South and Corewell Health West Laboratories 

Effective Date: February 3, 2025 

Human Papillomavirus (HPV) Genotypic testing will be updated to expand acceptable sources and collection methods. The following HPV test will be available, and the corresponding testing algorithm will be driven by the clinician specified specimen source and collection method/device.  

New Test: Self-Collect HPV High-Risk Screen with HPV Genotype Reflex (LAB1231853)  

  • Testing Restricted to HPV only –Co-testing Not Available: PAP or STI (CT, NG, TV)  
  • Detection of 14 HR HPV genotypes:  
    • HPV Genotype 16  
    • HPV Genotype 18
    • Other HR  HPV – includes 31,33,35,39,45,51,52,56,58,59,66,68  
  • Collection Information:  
    • Acceptable Source: Patient Self-collected vaginal  
    • Collection Device: Rovers® Evalyn® Brush  (ITM-1303902)
    • Collection Instructions: Patients must use the Evalyn Brush in a healthcare setting (i.e. physician office or clinic) to self-collect a vaginal specimen following the Evalyn Brush collection instructions. (Spanish instructions) 
    • A trained healthcare staff member must immediately suspend the Evalyn Brush into a vial of ThinPrep PreservCyt solution following transport instructions. The ThinPrep PreservCyt vial can then be transported to the lab at 2 to 30°C.  
    • This may not be collected at home or in an outpatient draw site. 

Corewell Health Southwest: Effective February 3, 2025, PAP and HPV results will once again be bundled for all orders placed from that date onward.

Update to PAP-HPV Reflexing Algorithm

The algorithm that reflexes HPV testing off the PAP Test (LAB1230097) when the “HPV Reflex” option is selected during PAP ordering will no longer include pregnancy status as a criterion for reflexing on LSIL patients. Prior to this update, if HPV Reflex was selected, a PAP Diagnosis of LSIL on patients 25 and older would only reflex HPV testing if the patient was non-pregnant. Starting 11/6/2024, if HPV Reflex was selected, a PAP Diagnosis of LSIL on patients 25 and older will reflex HPV testing regardless of pregnancy status. This update aligns with current ASCCP Risk-Based Management Consensus guidelines.

Hematology Testing Updates (Corewell Health West Michigan)

Effective Wednesday, July 30, 2025, changes to result reporting will be implemented across all Hematology Departments at Corewell Health Laboratories. These updates will help align all regions within Corewell Health Hematology Departments to provide a high-quality and standardize patient care experience.

The following are the most impactful changes for the Corewell Health West Michigan Region:

  • The Immature Platelet Fraction (IPF) will continue to be performed once every 24 hours if the platelet count is <100 x10^9/L, but the IPF results will now be located within the CBC.
  • The ‘immature cells’ component of the manual differential will be changed to ‘other cells’.
  • The Absolute Neutrophil Count (ANC) within a CBC w/Differential order will be EITHER an Automated ANC or a Manual ANC.
    • A CBC with Differential order with an automated differential will receive the automated ANC. A CBC with Differential order reflexing to a Manual Differential will receive a manual ANC.
  • The RBC Morphology will no longer include Anisocytosis, Microcytosis, or Macrocytosis components.
  • The RBC Morphology grading will change from “Moderate/Many” to “Present” for the following components:
    • Sickle Cells
    • Basophilic Stippling
    • Howell Jolly Bodies
    • Pappenheimer Bodies
    • Toxic Granulation
    • Dohle bodies
    • Vacuolated Neutrophils
    • Neutrophil Hypogranulation
    • Giant Platelets
  • The following RBC Morphology components will STILL be reported as Moderate/Many:
    • Hypochromia
    • Polychromasia
    • Schistocytes (Few, Moderate, Many)
    • Bite Cells
    • Spherocytes
    • Target Cells
    • Teardrop Cells
    • Acanthocytes
    • Blister Cells
    • Echinocytes
    • Macro Ovalocytes
  • Given the limited clinical utility and labor intensiveness of performing RBC counts, lower reportable limit for non-cerebrospinal fluid (non-CSF) body fluid red blood cell (RBC) counts will be updated to 2000 cells/uL. This means, if an RBC count on a non-CSF Body Fluid is <2000 cells/uL, Epic will report out the RBC count as <2000 cells/uL.
    • CSF will continue to report out all RBCs present within the cell count.

Reflex HPV Testing Update

Effective April 2, 2025, the Reflex HPV algorithm will more closely align with ASCCP recommendations. HPV testing will no longer be performed for a normal Pap result in patients aged 30 years and older when a provider orders HPV Reflex.

Specimens will qualify for HPV Reflex with a cytologic diagnosis of:

  • ASCUS (Atypical Squamous Cells of Undetermined Significance) for patients aged 21 and older.
  • LSIL (Low Grade Squamous Intraepithelial Lesion) for patients aged 25 and older.

HPV testing will continue to be performed for all Paps, regardless of results, for patients aged 30 years or older when a provider orders Co-Testing.

If a provider wishes to request HPV testing on a normal pap for patients under 30, they must either place an electronic order at the time of the initial pap order or request an add-on test for:

  • HPV High Risk Screen with HPV Genotype Reflex – Cervical Source (LAB1231770)
  • HPV High Risk Screen with HPV Genotype Reflex – Vaginal Source (LAB1230722)

Ordering Laboratory Supplies: Workflow Update and Updated Form (West Michigan)

West Michigan Corewell Health Laboratories and Supply Chain have updated the Laboratory Supply Requisition form and introduced a new workflow, for both Corewell Health and Non-Corewell Health offices. You may now email the completed form to CHLabSupplyRequisitions@CorewellHealth.org, although forms can still be sent with your courier if needed. This new workflow aims to improve communication between our lab courier team and your team members, reducing confusion and the time spent on phone calls for both parties.
