Human Papillomavirus (HPV) Testing Update

Corewell Health South and Corewell Health West Laboratories 

Effective Date: February 3, 2025 

Human Papillomavirus (HPV) Genotypic testing will be updated to expand acceptable sources and collection methods. The following HPV test will be available, and the corresponding testing algorithm will be driven by the clinician specified specimen source and collection method/device.  

New Test: Self-Collect HPV High-Risk Screen with HPV Genotype Reflex (LAB1231853)  

  • Testing Restricted to HPV only –Co-testing Not Available: PAP or STI (CT, NG, TV)  
  • Detection of 14 HR HPV genotypes:  
    • HPV Genotype 16  
    • HPV Genotype 18
    • Other HR  HPV – includes 31,33,35,39,45,51,52,56,58,59,66,68  
  • Collection Information:  
    • Acceptable Source: Patient Self-collected vaginal  
    • Collection Device: Rovers® Evalyn® Brush  (ITM-1303902)
    • Collection Instructions: Patients must use the Evalyn Brush in a healthcare setting (i.e. physician office or clinic) to self-collect a vaginal specimen following the Evalyn Brush collection instructions. (Spanish instructions) 
    • A trained healthcare staff member must immediately suspend the Evalyn Brush into a vial of ThinPrep PreservCyt solution following transport instructions. The ThinPrep PreservCyt vial can then be transported to the lab at 2 to 30°C.  
    • This may not be collected at home or in an outpatient draw site. 

Corewell Health Southwest: Effective February 3, 2025, PAP and HPV results will once again be bundled for all orders placed from that date onward.

Update to PAP-HPV Reflexing Algorithm

The algorithm that reflexes HPV testing off the PAP Test (LAB1230097) when the “HPV Reflex” option is selected during PAP ordering will no longer include pregnancy status as a criterion for reflexing on LSIL patients. Prior to this update, if HPV Reflex was selected, a PAP Diagnosis of LSIL on patients 25 and older would only reflex HPV testing if the patient was non-pregnant. Starting 11/6/2024, if HPV Reflex was selected, a PAP Diagnosis of LSIL on patients 25 and older will reflex HPV testing regardless of pregnancy status. This update aligns with current ASCCP Risk-Based Management Consensus guidelines.

Ordering Laboratory Supplies: Workflow Update and Updated Form (West Michigan)

West Michigan Corewell Health Laboratories and Supply Chain have updated the Laboratory Supply Requisition form and introduced a new workflow, for both Corewell Health and Non-Corewell Health offices. You may now email the completed form to CHLabSupplyRequisitions@CorewellHealth.org, although forms can still be sent with your courier if needed. This new workflow aims to improve communication between our lab courier team and your team members, reducing confusion and the time spent on phone calls for both parties.

Reminders: COVID Collection

Corewell Health Laboratories no longer offers Covid collection observations or collections at any of our draw sites/laboratories.

If your patient is experiencing symptoms and requires a Covid test, please choose one of the options below:

Collect the specimen in your office and send the specimen to any Corewell Health Laboratory or with your Corewell Health Courier, if available.

• Please be sure to place a collected order in Epic or your EMR, or include a paper order with the specimen.

• Please make sure the specimen is labeled with 2 patient identifiers, date and time of collection, and source (nasopharyngeal or nares)

NOTE: Walk-in COVID-19 testing is not available at any of our sites, including emergency departments, urgent care locations or walk-in clinics. Testing is available to registered patients who are seeking medical treatment.

Reminder: Laboratory Holiday Availability – Corewell Health West Michigan

COREWELL HEALTH OBSERVED HOLIDAY LIST:

  • New Year’s Day (January 1)*
  • Martin Luther King Jr, Day (3rd Monday in January)
  • Memorial Day (Last Monday in May)
  • Independence Day (July 4th)*
  • Labor Day (First Monday in September)
  • Thanksgiving Day (3rd Thursday in November)
  • Christmas Day (December 25th)*

† There may be varied hours for the Friday after Thanksgiving Day, and earlier closures for Christmas Eve (December 24th) and New Year’s Eve (December 31st). Please review site hours or contact site to ensure it is open.

If a holiday falls on a Saturday, it will be observed on the Friday before the holiday. If the holiday falls on a Sunday, it will be observed on the Monday after the holiday.

 

COURIERS

Laboratory specimen pickup and delivery schedules will be altered due to holiday observances.  To ensure specimen stability and prevent delays, follow these guidelines:

  • The couriers that support Corewell Health Laboratories in Grand Rapids (Kent County, Holland and Muskegon) will send a paper communication via your courier in the weeks leading up to the observed holiday, asking for information on if the office will be open and what the hours will be. Holiday routes are built around this information, a STAT courier will be available.
  • The couriers that support Corewell Health Laboratory Greenville Hospital in West Michigan will notify clients the week before a holiday via flier. An on-call STAT courier will be available on holidays. The phone number and name of the courier will be on the flier.
  • The couriers that support Corewell Health Laboratory Hospitals in the rest of West Michigan (Big Rapids, Gerber, Ludington, Pennock, Reed City, Zeeland) will not have courier pickup available. It is best to not schedule collections or to hold specimens at your facility if the specimen stability allows for it until your next available courier pickup.

Please note that the following tests should not be collected before an observed holiday due to FedEx shipping.

  • Please do not draw the following tests 2 days before through the observed holiday:

    • CMV T Cell Immunity Panel
    • Immuknow
    • Lymphocyte Mitogens and IABCS (Immune Assessment B Cell Subsets)
    • NK Cell Function to Cincinnati
  • Please do not draw the following tests 1 day before through the observed holiday:
    • TB Spots (Transplant TB Screen)
    • AlloMap
  • This does not include standard Mayo, Specialty or Viracor testing.

 

OUTPATIENT DRAW SITES AND ORDER ENTRY

Supply Change Notification: STI Testing

Beginning August 26, 2024, Corewell Health Laboratories (West and South) will begin updating instrumentation for STI and HPV testing. Aptima collection devices will no longer be available except for pediatric patients under 14 years old.

To order the new swabs:

  • Corewell Health and CHMG, please order via Workday ITM-1190747 as you run out of Aptima devices until the cutover date. (August 26th for West, late September for South)
  • Independent Offices or other non-Corewell Health Sites, please use the Supply Form and continue to order STD multi-collection kit or ThinPrep Pap Container.

New Test: HPV High Risk Screen with HPV Genotype Reflex

Effective August 26, 2024,  Corewell Health Laboratories West will be updating their High-Risk HPV testing.  The new assay will be performed on the Abbott Alinity m at Corewell Health Reference Laboratory West, Grand Rapids.  Testing will now automatically include HPV Genotype results whenever HPV is detected.  There will no longer be a separate charge to the patient for HPV Genotype testing.

There will also be an expanded number of High-Risk HPV Genotypes reported:

  • HPV Genotype 16
  • HPV Genotype 18
  • HPV Genotype 45
  • HPV Genotype 31/33/52/58
  • HPV Genotype 35/39/51/56/59/66/68

The new assay is FDA approved for Primary screening in addition to co-testing.

Post hysterectomy vaginal specimens will still be a send out test (LAB1230722). Continue Reading
