Effective October 18, 2026
To meet updated blood bank regulatory requirements, blood bank specimens must be collected using Positive Patient Identification (PPID) with electronic wristband barcode scanning whenever available.
Key changes:
• Electronic barcode scanning will replace current patient identification processes (B Number, Typenex band, and routine two-signature witness processes).
• Specimens collected without required electronic patient identification may be rejected and require recollection.
• For outpatient locations without barcode scanning capability, a two-signature witness process may be used.
• Patients without a historical ABO/Rh type on file may require an additional specimen before type-specific blood products can be issued.
• During planned or unplanned Epic downtime, the two-signature witness process must be followed.
