The apixaban level is a chromogenic assay for the quantitative determination of the concentration of apixaban in plasma. This assay is not indicated for monitoring the concentrations of low-molecular weight heparin (LMWH) or unfractionated heparin (UFH), and the presence of UFH and/or LMWH will cause the apixaban level to be falsely elevated. The analytical measuring range is 20-350 ng/mL. Therapeutic reference ranges have not been established, but observed median peak and trough levels per the 2021 International council for Standardization in Hematology recommendations are available (Douxfils et al. PMID:33742436). This testing is available daily for inpatient and outpatient use.
CLINICAL IMPLICATIONS
Apixaban is an oral anticoagulant that directly inhibits factor Xa and is approved by the US Food and Drug Administration for thrombosis prophylaxis in atrial fibrillation and surgical patients as well as for the treatment of venous thromboembolism (VTE). This medication does not require routine therapeutic monitoring, but measuring apixaban concentration can be useful in select clinical situations including: assessment of compliance, suspected overdose, periprocedural drug level, and in patients with conditions affecting pharmacokinetics such as renal insufficiency.
